15. Report any concerns you have about informed choices to the relevant person


Throughout any decision making process, if you have any concerns or are unsure of what to do, it is important to report these to the relevant person.

Record your concerns in the individual’s care plan and include any details on what action you have taken or going to take. Good communication and proactive reporting can help issues get resolved quicker and in an effective way. 

You can report concerns to:

A more experienced worker or your manager
A carer
A family member

Depending on the situation and what the choice is regarding, a senior member of staff, the individual’s carer or family member, may be the best person to speak to. You must not discuss individuals or their situations with your own family and friends; this information is confidential.

Speak to your manager if you have any concerns about the agreed ways of working on raising concerns and supporting individuals to make choices about their care.
