The level of monitoring, recording and reporting will depend on the individual and their personal circumstances.
Early detection of falls risks can lead to much better outcomes for people and can prevent injuries and psychological damage.
It is important to look out for hazards and risks and to know what to do if someone falls. It is also important you follow people’s support plans and falls risk assessments.
There will be agreed processes for this in your service, including monitoring charts. If you are unsure, then ask your supervisor for support.
If an individual has died, or has been injured because of a work-related accident, this may need to be reported under.

How and when to seek additional guidance
Additional guidance should be sought from the GP and or Falls Team if there are any concerns that an individual is losing their ability to mobilise after any fall. Also, if there are changes in behaviour that cause concern.
