16. Reporting medication usage problems


You may need to get further information when you are supporting a person or situation involving medication. 

This could be an individual having an adverse reaction to medication they are taking or it could be due to an error. 

Your role is to: 

Seek clarification if details or instructions are unclear 
Be vigilant and report any concerns or errors 
Inform your manager if the individual requests assistance or has any medication changes 

It is critical that you are honest and report any errors or concerns immediately. 

You may also need to get advice from the individual’s GP or a pharmacist. An investigation will be undertaken and systems may need to be changed so that the risk of errors in the future are reduced. 



Example – Pablo refuses to take his medication. 

You cannot legally force him to take his medication. Listen to why he is refusing. You can explain what his medication is for and you can give information about medication being available in different formats, which he might find easier to manage. 

If Pablo still refuses to take his medication, record on his MAR sheet and any daily records your organisation uses. Inform the person who will contact his GP for advice.

Do not be tempted to cover up mistakes or ignore concerns. 

It will put yourself and the individual at great risk if you do this!
