All of the needs that a person has must be discussed and documented in the care plan.

It is important to take a person-centred approach.

This means different information may be found in different people’s care plans. It is up to you to familiarise yourself with every care plan for each individual you work with. This means you will need to be flexibile and proactive.

Let’s look at Bob’s situation.

In the care plan, we would want to see guidance on what Bob can continue to do for himself in all areas of his life.
