3. The assessment process

As we mentioned in Lesson 2, information about each individual’s needs must be gathered.

It is important that the individual themselves is involved as much as possible in this assessment of needs process. The intention is for them to be empowered to communicate their wishes, by whichever means is possible for them.

Effective communication methods are critical.

Other people will also be important to the individual and their care plan. 

By involving all of the relevant people, you can gather a more holistic view of what is needed to meet an individual’s needs.

Read and flip the cards to identify some examples of people who might be important to inform an individual’s care plan:

Family members and carers
Advocates
A person holding a Lasting Power of Attorney (LPA)
Doctor
Social worker
Care manager
Dietician
Psychologist
Psychiatrist
Key worker
Representative from the care setting
Mental health nurse
Speech and language therapist (SALT)
Continence advisor

Each person will have a role in ensuring the best possible approach and outcomes are achieved for the individual and that a consistent approach to care and support is provided.
